This position is an exciting opportunity for a talented Registered Nurse to join our team at the Veterans Affairs, Veterans Health Administration. As a Nurse Care Specialist - Patient Education, you will play a vital role in providing high-quality patient care and education.
Job SummaryWe are seeking a skilled nurse with excellent communication and interpersonal skills to provide comprehensive patient education. The successful candidate will work closely with patients, families, and healthcare teams to develop personalized education plans that promote health literacy and empower individuals to manage their healthcare effectively.
Responsibilities- Deliver safe and effective patient care, prioritizing individual needs and circumstances.
- Develop and implement patient education plans tailored to each individual's learning style and goals.
- Maintain accurate records of patient interactions, education provided, and outcomes achieved.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
- Stay up-to-date with the latest evidence-based practices, guidelines, and research in patient education.
- Current, active, and unrestricted registration as a graduate professional nurse in any state or territory of the United States.
- At least 1 year of experience in patient education or a related field.
- Strong knowledge of adult learning principles, health literacy, and patient-centered care.
- Excellent verbal and written communication, interpersonal, and problem-solving skills.
- Able to work independently and collaboratively within an interdisciplinary team environment.
- Competitive salary: $65,000 - $85,000 per annum (dependent on experience).
