Job Description:

We are seeking an experienced and compassionate Hospice Care Manager to join our team at St. Croix Hospice. As a key member of our interdisciplinary group, you will provide high-quality patient care and support to individuals and families in their homes and communities.

The ideal candidate will have a strong background in nursing, with emphasis on community health education and experience working with hospice patients. You will be responsible for organizing and directing hospice care, providing professional nursing services, and ensuring the highest level of patient satisfaction.

The salary range for this position is $85,000 - $110,000 per year, depending on experience. We offer a comprehensive benefits package, including medical, dental, vision, and life insurance, as well as paid time off and holidays.

Responsibilities:

  • Provide high-quality patient care and support to individuals and families in their homes and communities
  • Organize and direct hospice care, including developing and implementing individualized care plans
  • Collaborate with the interdisciplinary group to ensure seamless care coordination and communication
  • Assess and evaluate patient status, identifying areas for improvement and developing strategies for enhanced care
  • Document patient information accurately and thoroughly, maintaining up-to-date records
  • Foster relationships with patients, families, and caregivers, providing emotional support and guidance as needed

Requirements:

  • Bachelor's degree in nursing or related field
  • At least two years of experience in hospice care or a related field
  • Current licensure in the state of practice
  • CPR certification
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
