Job Summary:

We are seeking an experienced Admissions Nurse to join our team at St. Croix Hospice. This is a full-time position that requires strong nursing skills, excellent communication abilities, and a passion for delivering high-quality patient care.

About the Role:
  • This professional nurse will be responsible for planning, organizing, and directing hospice care services for patients in their homes and communities.
  • The successful candidate will have experience in community health education and will build on existing resources to meet the needs of individuals and families within their homes and communities.
Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Provide professional nursing care by utilizing all elements of the nursing process.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care.
Requirements:
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting.
  • Current licensure in State and CPR certification.
  • Associates degree required, bachelor's degree preferred.
  • Experience in hospice care and home care is highly desirable.
