Job Title: Licensed Practical Nurse, PRN

ClearSky Health is seeking a skilled Licensed Practical Nurse to join our team in an inpatient hospital setting. As a key member of our healthcare team, you will provide high-quality patient care and contribute to the delivery of exceptional patient experiences.

Job Summary:

The Licensed Practical Nurse will be responsible for providing general nursing care to patients, under the direction of a Registered Nurse and in accordance with physician's orders and established standards and protocols. This role requires a strong commitment to patient-centered care, effective communication, and teamwork.

Key Responsibilities:
  • Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
  • Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
  • Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
  • Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
  • Reports to supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
Requirements:
  • Two years' experience in inpatient hospital setting, inpatient rehabilitation experience preferred.
  • Current state license as a Licensed Practical Nurse required.
  • Current CPR certification required.
Preferred Qualifications:
  • Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
  • Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
  • Effective organizational and time management skills.
  • Effective written and verbal communication skills.
Physical Requirements:

A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time. Both gross and precise motor functions. Lifting/exerting of up to 50 lbs. Possible exposure to bodily fluids. Visual acuity required for patient assessment and documentation of care. Acute hearing required for accurate patient assessment. Sufficient manual dexterity to operate equipment and computer keyboard. Close vision and the ability to adjust focus.
