Hospice Registered Nurse Job Description

At Home Care Connectors, we are seeking a compassionate and skilled Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will provide high-quality nursing care to terminally ill patients and their families in the comfort of their own homes.

Key Responsibilities:
  • Assess and reassess the nursing needs of patients and develop individualized care plans
  • Provide education and support to patients and their families on end-of-life care and symptom management
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination
  • Document patient progress and maintain accurate records
  • Participate in on-call rotation and provide emergency care as needed
Requirements:
  • Current RN licensure in the state of Connecticut
  • Minimum 2 years of experience in hospice or palliative care
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Strong assessment and problem-solving skills
