Job Summary

We are seeking a compassionate and dedicated Hospice Nurse to join our team at Home Care Connectors. The successful candidate will provide end-of-life care and support to patients and their families in a hospice setting.

Key Responsibilities:

  • Oversee the clinical management of the Agency's clinical department, ensuring high-quality care and adherence to policies and procedures.
  • Assist the Administrator in planning the overall development and administration of the Agency, aligning with organizational goals and objectives.
  • Participate in the development of administrative policies and procedures, ensuring compliance with regulatory requirements and industry standards.
  • Direct the implementation of approved work methods and procedures, promoting a culture of excellence and continuous improvement.
  • Participate in the review and analysis of the Agency's effectiveness, identifying areas for improvement and implementing strategies to enhance patient care and outcomes.
  • Utilize data reports to quantify the Agency's performance against national norms, recommending methodologies to improve outcomes and OBQI scores.
  • Oversee the implementation and evaluation of patient care services, ensuring timely and effective delivery of care.
  • Aggregate record review summaries, providing insights into patient care and outcomes.
  • Collaborate with the recruiter to achieve satisfactory staffing levels, promoting a culture of excellence and high-quality care.
  • Assure that established QA committees maintain the goal of meetings, promoting a culture of continuous improvement.
  • Assist the Administrator in planning and conducting annual Agency requirements, ensuring compliance with regulatory requirements and industry standards.
  • Assist the Administrator in collecting data for the Annual Agency Evaluation and Annual Report, providing insights into Agency performance and outcomes.
  • Recognize leadership potential and offer opportunities for leadership training and development, promoting a culture of excellence and continuous improvement.
  • Manage the clinical departmental budget, seeking supplementals prior to exceeding budgetary requirements.
  • Evaluate own job performance and utilization of resources, planning for professional growth and development.
  • Receive and act on complaints, subject to the agency's grievance procedure, and apprise the Administrator of potential legal risks.
  • Participate in employee termination processes, ensuring documentation exists to support actions.
  • Ensure the implementation of the Agency's continuing education program, promoting a culture of excellence and continuous improvement.
  • Project a concerned, professional attitude and develop a positive rapport with all staff, promoting a culture of excellence and high-quality care.
  • Establish mutual goal setting and achievement of standards, promoting a culture of excellence and continuous improvement.
  • Develop an open, positive rapport with community resources affiliated with the Agency, promoting a culture of collaboration and partnership.
  • Maintain high visibility and availability while in the office, promoting a culture of accessibility and responsiveness.
  • Assist the Sales/Marketing department as requested, promoting a culture of collaboration and partnership.
  • Serve as a member of the QA/Advisory/Emergency Planning and other committees, promoting a culture of excellence and continuous improvement.
  • Participate in local organizations and activities related to home health care, promoting a culture of collaboration and partnership.
  • Participate in state and national organizations, meetings, seminars, workshops, and activities, promoting a culture of excellence and continuous improvement.
  • Accommodate state/accreditation surveyor/staff during site visits, promoting a culture of compliance and regulatory adherence.
  • Oversee the assignment of the Primary case manager for patient admissions, ensuring timely and effective delivery of care.
  • Other duties as assigned, promoting a culture of flexibility and adaptability.

Requirements:

  • Valid RN license in the state of practice.
  • Previous experience in management.
  • Knowledge of Athenahealth or similar electronic medical record systems.
  • Familiarity with medical office standards and procedures.
  • Understanding of behavioral health principles.
  • Experience in utilization management and occupational health.
  • Proficiency in airway management techniques.
  • Skilled in administering injections.
