**Job Summary:**
We are seeking a compassionate and dedicated Hospice Nurse to join our team at Home Care Connectors. The Hospice Nurse will provide end-of-life care and support to patients and their families in a hospice setting.
Key Responsibilities:
- Oversee the clinical management of the Agency's clinical department, ensuring high-quality care and adherence to policies and procedures.
- Assist the Administrator in planning the overall development and administration of the Agency, aligning with established goals and objectives.
- Participate in the development of administrative policies and procedures, ensuring they reflect best practices in home health care.
- Direct the implementation of approved work methods and procedures, focusing on elements essential to rendering high-quality care.
- Participate in the review, analysis, and appraisal of the effectiveness of the total Agency program, identifying areas for improvement.
- Utilize OBQI/OBQM data reports to quantify Agency performance against national norms, recommending strategies to enhance outcomes and improve OBQI scores.
- Oversee the implementation and evaluation of patient care services, ensuring they meet Agency standards and regulatory requirements.
- Aggregate record review summaries, providing insights for quality improvement initiatives.
- Collaborate with the recruiter to achieve satisfactory staffing levels, promoting high-quality care and maintaining standards.
- Ensure established QA committees maintain their goals and objectives, fostering a culture of continuous improvement.
- Assist the Administrator in planning and conducting annual Agency requirements, ensuring compliance with regulatory standards.
- Assist the Administrator in collecting data for the Annual Agency Evaluation/Annual report, providing valuable insights for strategic planning.
- Recognize leadership potential and offer opportunities for leadership training and development, empowering staff to grow professionally.
- Manage the clinical departmental budget, seeking supplements prior to exceeding budgetary requirements.
- Evaluate own job performance and utilization of resources, planning for professional growth and development.
- Receive and act on complaints, adhering to the agency's grievance procedure, and apprise the Administrator of potential legal risks.
- Participate in the employee termination process, ensuring documentation supports actions taken.
- Ensure implementation of Agency continuing education programs, promoting staff development and knowledge sharing.
- Project a concerned, professional attitude and develop a positive rapport with all staff, fostering a collaborative work environment.
- Establish mutual goal setting and achievement of standards, promoting a culture of excellence.
- Develop an open, positive rapport with community resources affiliated with the Agency, building partnerships for improved patient care.
- Maintain high visibility and availability while in the office, ensuring prompt response to staff and patient needs.
- Assist the Sales/Marketing department as requested, supporting business development initiatives.
- Serve as a member of the QA/Advisory/Emergency Planning & other committees, contributing to Agency decision-making processes.
- Participate in local organizations and activities related to home health care, staying informed about industry trends and best practices.
- Participate in state and national organizations, meetings, seminars, workshops, and activities, expanding knowledge and expertise.
- Accommodate state/accreditation surveyors/staff during site visits, ensuring a smooth and compliant experience.
- Oversee the assignment of the Primary case manager for patient admissions, ensuring patient needs are met.
- Perform other duties as assigned, supporting the Agency's mission and goals.
