Job Overview

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team in Newington, Connecticut.

About the Role

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment. Based on this assessment and evaluation, the Occupational Therapist will help determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Key Responsibilities
  • Provides occupational therapy services within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the healthcare team.
  • Evaluates patients' functional status, body systems, and occupational therapy needs. Consults with physicians to develop therapy plans of care.
  • Observes, records, and reports patients' responses to treatment and changes in their condition to nurse supervisors and physicians.
  • Instructs and informs healthcare team members, patients, and family/caregivers about the plan of care, functional limitations, and progress towards goals.
  • Visits patients throughout the service area and other nearby LHC agencies as needed.
  • Completes progress notes for each visit and submits them per LHC policy.
  • Develops OT plans of care within state-specific guidelines and reports outcomes, goals, and anticipated frequency of care to nurse supervisors within 24 hours.
  • Participates in staff conferences and committees as requested.
  • May supervise home health aides per LHC policy if allowed by state regulation.
  • Provides supervision of OTA personnel according to LHC policy and state regulations.
Requirements

  • A current Occupational Therapy licensure in the state of employment.
  • A valid CPR certification.
  • A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.

Estimated salary: $83,000 - $115,000 per year, based on experience and qualifications. This figure may vary depending on individual circumstances.
