This professional is responsible for verifying medication orders, selecting appropriate drug products, designing dosage regimens, and providing comprehensive drug therapy monitoring services. Collaboration with patients, families, and healthcare teams is essential to guarantee continuity of care and achieve optimal patient outcomes through efficient and safe medication management.
Key Responsibilities:
- Oversee the medication order verification process and assess drug product selections.
- Design and implement effective dosage regimens tailored to individual patient needs.
- Monitor drug therapy and provide guidance to healthcare providers and patients.
- Supervise pharmacy technicians and manage pharmacy operations in the absence of management.
- Lead pharmacy services in designated clinical or operational areas.
Educational Requirements:
- Graduation from an ACPE-accredited School of Pharmacy - Required
- Completion of an ASHP-accredited General Practice Residency (PGY1) - Preferred
Experience Requirements:
- Experience in healthcare settings, particularly in infusion pharmacy - Preferred
- Completion of an ASHP-accredited Residency in Pharmacy (PGY1) - Preferred
Additional Qualifications for Oncology Pharmacy:
- Experience in hematology/oncology is strongly preferred.
- Knowledge of solid tumors, hematology, and standard treatment protocols.
- Able to educate and monitor patients undergoing chemotherapy.
- Prior experience in inpatient pharmacy or infusion center environments.
- Flexibility to work various shifts, including weekends and holidays.
Licensure and Certifications:
- Registered Pharmacist (RPH) - Required
- Basic Life Support (BLS) certification - Required
- Board Certified Oncology Pharmacist (BCOP) - Preferred
- Advanced Cardiac Life Support (ACLS) certification - Preferred
- Pediatric Advanced Life Support (PALS) certification - Required for specific units.
- Advanced Practice Pharmacist License (APh) - Preferred
Work Schedule:
This position requires flexibility in working various shifts, including evenings and weekends.
Compensation:
Competitive hourly rate based on relevant experience and licensure.
Weekly Hours:
40 hours per week.
