We are seeking a highly skilled and compassionate Registered Nurse to join our team at WVUHS Home Care, LLC. As a Registered Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes. This is a fantastic opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide direct and indirect nursing care to patients in their homes, including assessing their needs, developing care plans, and implementing interventions.
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care and achieve optimal patient outcomes.
- Conduct thorough assessments, develop and implement individualized care plans, and evaluate patient progress.
- Administer medications, perform treatments, and provide education to patients and their families.
- Document patient information accurately and thoroughly, including progress notes, medication administration records, and other relevant data.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in ongoing education and professional development to stay current with best practices and industry advancements.
- Current licensure as a Registered Nurse in the state of West Virginia or a compact state.
- Valid driver's license and reliable transportation.
- CPR certification within 30 days of hire date.
- One year of home care nursing experience and two years of prior nursing experience.
- IV skills and experience with home health care.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
This position requires frequent travel to patients' homes, which may involve exposure to various environments and situations. The Registered Nurse must be able to work effectively in a fast-paced environment, prioritize tasks, and maintain a high level of professionalism and compassion.
