Job Overview

We are seeking a dedicated Care Coordinator to become a vital member of our team at the Center for Family Health & Education. As a Care Coordinator, you will play a critical role in ensuring the seamless delivery of medical and health services to our patients.

Responsibilities:

  • Develop and implement plans to improve the efficiency and effectiveness of healthcare services.
  • Collaborate with healthcare professionals to design and implement programs that meet the unique needs of patients and families.
  • Conduct regular audits to ensure compliance with healthcare regulations and policies.
  • Provide training and support to staff to enhance their skills and knowledge in healthcare delivery.
  • Participate in quality improvement initiatives to identify areas for improvement and develop solutions.

Qualifications:

  • A bachelor's degree in healthcare administration or a related field.
  • At least 3 years' experience in a healthcare setting, preferably in a supervisory or management role.
  • Proven leadership skills, with the ability to motivate and guide staff.
  • Exceptional analytical and problem-solving skills, with the ability to think critically and make informed decisions.
