About the Role

At UnitedHealthcare, we're committed to simplifying the healthcare experience and creating healthier communities. As a Field Health Social Services Coordinator/IDD, you'll play a vital role in removing barriers to quality care and making a meaningful impact on people's lives.

Key Responsibilities
  • Develop and implement individualized care strategies that prioritize the most appropriate level of care for each patient.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care, serving as a single point of contact and ensuring seamless communication with stakeholders.
  • Advocate for patients and families as needed, ensuring their needs and choices are represented and supported by the healthcare team.
Requirements
  • Licensed Master's Social Worker (LMSW), Licensed Professional Counselor (LPC), or Licensed Bachelor Social Worker (LBSW) OR RN in the state of Texas.
  • 2+ years of experience working directly with people with Intellectual and Developmental Disability (IDD).
  • 2+ years of experience working within the community health setting or in a healthcare role.
  • Familiarity with Microsoft Office, including Word, Excel, and Outlook.
  • Reliable transportation and the ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
  • High-speed internet in residence.
Preferred Qualifications
  • 1+ years of experience with long-term care services and support, Medicaid, or Medicare.
  • Ability to create, edit, save, and send documents, spreadsheets, and emails.
  • Knowledge of the principles of most integrated settings, including federal and state requirements.
