Job Summary:

We are seeking a skilled Social Worker to join our team at Loyola University Health System. As a Social Worker - Case Management, you will play a critical role in providing exceptional care to our patients and their families.

Key Responsibilities:
  • Assess and provide intervention concerning disability resulting from emotional, social, and economic stresses of illness.
  • Identify and provide interventions to meet the psychosocial needs of patients and their families.
  • Resource for complex discharge planning needs.
  • Collaborate with healthcare teams to develop and implement comprehensive care plans.
  • Provide education and support to patients and families to promote healthy behaviors and self-care.
Requirements:
  • Master's Degree in Social Work.
  • Minimum of six (6) months of prior social work experience in a hospital setting.
  • Current certification by the State of Illinois as a Licensed Social Worker (L.S.W.).
  • Ability to take call and work flexible hours including weekends.
