Social Worker

We are seeking a skilled Social Worker to join our team at Loyola University Health System. As a Social Worker, you will play a vital role in providing exceptional care to our patients and their families. Your expertise in biopsychosocial assessment, case management, and patient navigation will enable you to make a meaningful impact in the lives of those we serve.
Key Responsibilities- Assess and manage mental health conditions, such as depression and anxiety, in patients and their families.
- Provide case management and care coordination services, particularly for individuals with chronic and complex medical conditions.
- Navigate patients through the healthcare system, ensuring seamless transitions between inpatient, outpatient, home health, and long-term care settings.
- Counsel patients on adjustment to chronic disease, life planning, and end-of-life issues.
- Identify and refer patients to specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
- Develop and implement education and support programs for individuals and groups, addressing topics such as diabetes education, parenting classes, and domestic violence support.
- Assist patients with entitlements, medications, transportation, and advance directives.
- Assess and intervene in domestic violence and child abuse situations.
- Collaborate with community resources and agencies, including our internal Community Health Worker team, to provide comprehensive care.
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure as a Licensed Social Worker (L.S.W.) in the State of Illinois.
- Minimum of six (6) months of prior social work experience in a hospital setting.
- Familiarity with Joint Commission standards.
- Excellent interpersonal, analytical, and communication skills.
- Ability to work independently, prioritize complex case loads, and manage multiple responsibilities.
