Social Worker

We are seeking a skilled Social Worker to join our team at Loyola University Health System. As a Social Worker, you will play a vital role in providing exceptional care to our patients and their families.
Key Responsibilities- Assess and provide intervention concerning disability resulting from emotional, social, and economic stresses of illness
- Identify and provide interventions to meet the psychosocial needs of patients and their families
- Resource for complex discharge planning needs
- Utilize established practice and professional standards to assess and provide interventions
- Collaborate with healthcare teams to develop and implement comprehensive care plans
- Provide education and support programming for individuals and groups
- Assist with entitlements, medications, transportation, and advance directives
- Assess and intervene in domestic violence and child abuse situations
- Outreach and coordination with other community resources and agencies
- Master's degree in Social Work (MSW) from an accredited program
- Current certification by the State of Illinois as a Licensed Social Worker (L.S.W.)
- Minimum of six (6) months of prior social work experience in a hospital setting
- Familiarity with Joint Commission standards
- Excellent interpersonal, analytical, and communication skills
- Ability to work independently and as part of a team
- Flexibility to take call and work flexible hours, including weekends
