Job Description

OVERVIEW

The Refugee Wellness Program is seeking a skilled Case Manager to provide support to refugees in their community integration and self-sufficiency journey. The ideal candidate will work collaboratively with local partners, make referrals to community services, and provide transportation to clients as needed.

Key Responsibilities:

  • Provide case management services to refugees, focusing on health, wellness, integration, and self-sufficiency.
  • Develop personalized self-sufficiency plans for clients.
  • Conduct referrals to agency services to remove barriers to self-sufficiency.
  • Maintain accurate case files and databases.
  • Submit written reports as required.
  • Provide cross-cultural information and support to community stakeholders.
  • Orient refugee clients to community resources.
  • Connect clients with supportive community networks and systems for long-term integration.
  • Ensure refugees receive linguistically and culturally appropriate services, providing education and advocacy when necessary.

Requirements:

  • Bachelor's degree in social work, human services, public health, or a related field.
  • Prior experience in case management or social work is preferred.
  • Bilingual or multilingual skills in languages commonly spoken by refugee communities are a plus.
  • Ability to work as a team member and independently, with high self-motivation.
  • Excellent communication skills, with good written English command.
  • Exceptional interpersonal skills, including keen listening ability and relationship building.
  • Proficient in Microsoft Office.
  • Must pass a criminal background check and other screening requirements.
  • Must have a valid driver's license, insurance, and a clean driving record.

Training and Development:

  • Satisfactory completion of USCRI's Orientation and Training.
  • Additional training as identified by the supervisor or Human Resources.

Physical Demands:

  • Manual dexterity, tactile, visual, and audio acuity.
  • Repetitive motion, prolonged sitting and standing, and sustained visual and mental applications.
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying.
