Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to seniors in their homes.

About the Role:

This is a field-based position, requiring daily driving and travel to patient homes in Alpharetta, Cumming, and Cherokee, GA. You will work independently, using your advanced education and clinical competencies to achieve optimal patient outcomes.

Responsibilities:
  • Provide health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions.
  • Prescribe medication, examine patients, diagnose illnesses, and provide treatment.
  • Advise executives on matters of significance, exercising independent judgment and decision-making on complex issues.
  • Work under minimal supervision, using independent judgment to determine the best course of action.
Requirements:
  • Valid state driver's license, clean driving record, and proof of auto insurance.
  • Bachelor's degree and minimum 2 years clinical experience.
  • Active/unrestricted state license, DEA license, and board certification.
  • Senior patient care experience and strong technical experience.
  • Active BLS through the AHA or ARC.
  • Strong belief in the revolutionary nature of house calls and passion for improving care delivery.
Preferred Qualifications:
  • Master's in science of nursing.
  • House call experience.
  • Urgent care, primary care, family practice, or internal medicine experience.
  • Value-based care knowledge and/or experience.
  • HEDIS quality metrics knowledge and/or experience.
