Job Summary

CenterWell Senior Primary Care is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a Field-Based Nurse Practitioner, you will provide high-quality, patient-centered care to seniors in their homes, focusing on proactive, preventive care and addressing the physical, emotional, and social well-being of our patients.

Key Responsibilities
  • Provide comprehensive primary care services to seniors in their homes, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
  • Develop and implement personalized care plans to address the unique needs and goals of each patient.
  • Collaborate with our integrated care teams, including physicians, nurses, and behavioral health specialists, to ensure seamless care coordination and optimal patient outcomes.
  • Stay up-to-date with the latest clinical guidelines and best practices in primary care, and apply this knowledge to improve patient care and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of new care models and programs.
Requirements
  • Master's degree in Nursing or related field.
  • Active, unrestricted state license and DEA license.
  • Board certification as a Nurse Practitioner.
  • Minimum 2 years of clinical experience, preferably in primary care or gerontology.
  • Strong technical skills, including proficiency in electronic medical records and HIPAA-secure messaging apps.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Passion for delivering high-quality, patient-centered care and a commitment to improving the health and well-being of seniors.
Preferred Qualifications
  • House call experience or experience working in a similar field-based setting.
  • Urgent care, primary care, family practice, or internal medicine experience.
  • Value-based care knowledge and/or experience.
  • HEDIS quality metrics knowledge and/or experience.
