About the Role:

The Patient Care Coordinator RN will be responsible for coordinating and managing patient care services, ensuring seamless transitions between healthcare settings. Key Responsibilities:
  • Coordinate patient care services, including assessments, planning, implementation, and evaluation
  • Develop and maintain comprehensive patient plans, taking into account medical, biopsychosocial, behavioral, spiritual, and cultural needs
  • Collaborate with patients, caregivers, and healthcare providers to identify and address barriers to care
  • Implement patient-centered interventions, set goals, and develop plans of care based on transitional care discharge plans and comprehensive assessments
  • Facilitate education and support for patients and families, promoting self-management and empowerment
  • Maintain accurate and timely documentation in electronic health records, adhering to organizational policies and procedures
  • Engage with community provider agencies to promote quality care and cost-effective health outcomes
Requirements:
  • Associate degree or better from an accredited School of Nursing with an unrestricted license to practice nursing in NC
  • Certified Case Manager (CCM) preferred
  • Excellent written and verbal communication skills, with ability to work effectively with diverse populations
  • Proficiency in Microsoft Office Outlook, Word, Excel, PowerPoint, and e-mail communication
Salary and Benefits:

This is a full-time position with a salary range of $60,000 - $80,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package, including health insurance, retirement plan, paid time off, and professional development opportunities.

Work Environment:

This role may require occasional travel, up to 50% of the time. The ideal candidate will have reliable transportation and a valid NC driver's license. Regular attendance and flexibility to work in a fast-paced environment are essential.
