About the Role

We are seeking a skilled Hospice Social Worker (MSW) to join our interdisciplinary team at Heartland Hospice. As a key member of our team, you will provide essential psychosocial support to patients and their families, utilizing your expertise in social work and your passion for delivering high-quality care.

Responsibilities
  • Conduct comprehensive psychosocial assessments to identify the needs, goals, and interventions for patients and their families.
  • Develop and implement individualized plans of care that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on end-of-life care, advance care planning, and bereavement.
  • Participate in ongoing professional development to stay current on best practices in hospice care and social work.
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school.
  • Licensure as required by the state in which the hospice is located.
  • At least one year of experience as a social worker in a healthcare and/or hospice setting, with three to five years of experience preferred.
  • Knowledge of terminally ill patients and their families, as well as an understanding of hospice care and the psychosocial dynamics of illness, loss, and death.
