We are seeking a dedicated and compassionate Service Coordinator to join our team at Service Access & Management, Inc. This role is a great opportunity for individuals who are passionate about making a difference in the lives of individuals who need long-term support and services.


Key Responsibilities:

As a Service Coordinator, you will provide essential support to individuals involved with Long Term Support and Services, helping them navigate and access the resources they need. Your responsibilities will include:


  • Assessing the needs of participants and completing comprehensive assessments.
  • Linking participants to needed and/or approved services.
  • Developing and maintaining plans that outline participant strengths and needs.
  • Providing ongoing monitoring activities to ensure adherence to plans and satisfaction with services.
  • Communicating priorities and information to participants and their identified team members.

Requirements:

To be successful in this role, you will need:


  • A bachelor's degree in Social Work, Psychology, or a related field, or a Registered Nurse (RN) degree.
  • At least three (3) years of experience in a social service or healthcare-related setting.
  • Valid Pennsylvania driver's license and automobile insurance.
  • Acceptable Motor Vehicle Record Check and Act 33 and Act 34 Clearances.
  • FBI Fingerprint Clearance and Social Security Card.

Service Coordinators providing services within the Nursing Home project must complete TB testing.


Working Conditions:


Comfortable working within an open office environment and communicating through virtual technology. Travel throughout the coverage area is required and may occur in adverse conditions. Must be able to lift at least 15 pounds.
