Job Title: Clinical Case Manager - Intensive Services

Job Summary:

The Clinical Case Manager - Intensive Services will work with a diverse and marginalized client caseload to provide intensive field-based case management and clinical services. This role requires a strong understanding of trauma-informed care and harm reduction approaches, as well as the ability to work collaboratively with clients and other stakeholders to achieve positive outcomes.

Key Responsibilities:
  • Assess mental health service needs of clients and provide therapeutic services or link to appropriate entities
  • Provide crisis intervention and safety assessments to clients on caseload
  • Link clients with appropriate medical, mental health, substance abuse, and vocational resources
  • Assist clients in obtaining all benefits to which they are entitled
  • Develop and maintain a complete, accurate, and current client file with all required documents and data in agency records and electronic databases
  • Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations
  • Transport clients in your vehicle to various agencies and healthcare providers in the greater Los Angeles area to increase community support services and community reintegration
Qualifications:
  • MSW, MFT or LPC who is BBS eligible (supervision hours are available for this position)
  • Registered with the board of behavioral sciences or ability to do so within the first 2 months of employment
  • Demonstrated knowledge of clinical case management
  • At least one year of experience working with clients experiencing homelessness and with mental illness, chronic health issues, and substance use disorders
  • Knowledge of community resources
  • Able to work in a high-tolerance, harm reduction model with individuals who have multiple barriers
  • Strong clinical, team-building, charting, and non-violent crisis intervention skills
  • Computer literate; able to effectively use a computerized database for client file management, with necessary skills in Microsoft Word, Outlook, and Excel programs
  • Detail-oriented with strong time management, organizational, written, verbal, critical thinking and interpersonal skills
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record
  • Able to transport clients in your vehicle
Work Environment:

This role requires a combination of field and office environment, with regular travel to different local sites. The successful candidate will be able to work in a busy and loud environment, and will be exposed to elements like cold, heat, dust, noise and odor.
