About the Role

We are seeking a highly motivated and compassionate Wellness Program Coordinator to join our team at Hebrew SeniorLife, Inc. This is a 24-hour per week, on-site position that requires a strong commitment to supporting the health and well-being of our senior residents.

Key Responsibilities
  • Provide intensive case management services to residents, including conducting wellness assessments, developing wellness plans, and coordinating with healthcare providers.
  • Partner with housing staff to tailor programming that meets resident needs, including promoting self-care and connecting residents to community resources.
  • Coordinate with residents to complete files, including important health care information, end-of-life planning, and emergency contacts.
  • Communicate with healthcare providers to relay important information and ensure seamless care transitions.
  • Assess the needs of residents with dementia and make appropriate referrals as needed.
  • Collaborate with housing staff to ensure residents are connected with all assistance programs for which they are eligible.
  • Work with local hospitals and home care agencies to ensure safe discharges and coordinate care planning.
  • Offer advocacy for residents and serve as a liaison for families.
  • Document all work in online database.
Requirements
  • Bachelor's degree with 3-5 years of experience in a human service field, preferably in aging services.
  • Excellent organization and interpersonal skills, with ability to innovate, think strategically, and manage multiple projects simultaneously.
  • Computer literacy, with experience in Windows, Word, PowerPoint, and Excel.
  • Preferred qualifications include a Master's degree in a human service field and 1-2 years of experience working as a case manager or care coordinator in aging services, as well as certification in dementia care.
