LPN Care Coordination Job Description

We are seeking a skilled LPN to join our team as a Care Coordinator. In this role, you will play a vital part in ensuring the seamless delivery of patient care.

Key Responsibilities:
  • Update and maintain electronic patient records, transitioning hard copy records to digital format
  • Record patient information, including vital signs, weight, and changes in medical history before each appointment
  • Clean and prepare examination rooms before patient appointments
  • Work in the front office, handling tasks such as answering phone calls and greeting patients
  • Assist in maintaining medical inventory and placing orders for new materials as needed
  • Measure and record vital signs
  • Record patient interviews, history, and chief complaints
  • Provide patient education on medications, disease management, home treatments, and special diets
  • Prepare patients for examinations and perform routine screening tests
  • Assist physicians with exams
  • Perform phlebotomy and collect lab specimens
  • Perform basic lab tests and EKGs
  • Administer and document medication under a physician's order
Requirements:
  • Current NYS LPN license required
  • Preferred Bilingual English/Spanish speaking
  • Preferred Urology/Surgical experience
  • Basic Life Support (BLS) certification required
  • Previous experience in a lead role or demonstrated potential to act in a lead capacity
  • Consistent professional conduct and meticulous attention to detail
