Position Details

We are seeking a responsible and patient Licensed Practical Nurse to join our team as a Care Coordinator. This role involves reviewing patient charts to ensure accuracy and completeness of records, as well as submitting Review Visit Logs (RVLs) to supervisors. The successful candidate will also be responsible for adhering to operational guidelines, responding to correspondences, and attending educational meetings.

Key Responsibilities
  • Review patient records to ensure accuracy and completeness
  • Submit Review Visit Logs (RVLs) to supervisors
  • Adhere to operational guidelines for color coding of patients
  • Respond to HR, operational, and clinical correspondences
  • Submit clinical corrections to supervisors
  • Attend all operational and educational pod meetings as scheduled
  • Promptly communicate with Operations to resolve IT issues
  • Cross-train in other areas of care management

Salary Range

$60,000.00 - $65,000.00 per year
