About The Renaissance Community

The Renaissance Community is a supportive living program serving individuals in Montgomery County. We offer 24-hour supervision in a community setting with an emphasis on behavioral health services. Individuals reside in either communal homes or apartments, tailored to their needs for structure and supervision. Our programs encourage participation in activities, groups, day treatment programs, and medication monitoring to support personal growth and independence.

Position Summary

As a Clinical Care Coordinator, you will play a vital role in organizing patient care activities and facilitating communication among all stakeholders involved in a patient's treatment. Your responsibilities will encompass program development, monitoring, and ensuring comprehensive care plans are implemented effectively.

Essential Duties and Functions

  • Develop and monitor comprehensive treatment plans for individuals requiring services across multiple settings.
  • Collaborate with internal and external providers to coordinate care and ensure seamless transitions.
  • Maintain up-to-date knowledge of available community resources, government regulations, and program funder requirements.
  • Facilitate regular treatment team meetings to discuss progress, challenges, and adjustments to care plans.
  • Track client outcomes and provide reports on the quality of care delivered.

Qualifications

  • Master's Degree in a Human Service field.
  • One to two years of experience working with individuals with mental health diagnoses, intellectual and developmental disabilities, or substance use disorders.
  • Valid Driver's License required for travel within the program service area.
