Key Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services
  • Identify and address the physical, psychosocial, and environmental needs of patients and their families
  • Develop and implement individualized plans of care, collaborating with the patient, family, and healthcare team
  • Provide ongoing assessment and evaluation of patient progress, making adjustments to the plan of care as needed
  • Document patient information, including problems, goals, interventions, and outcomes
Requirements
  • Current RN licensure in the state of practice
  • One year of clinical experience
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
