Job Summary

The Registered Nurse Case Manager is a key member of the Arkansas Hospice team, responsible for providing high-quality patient care and support to individuals and their families. This role requires a strong nursing background, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Responsibilities

• Manages patient care functions after initial admission and assignment to a designated RN Case Manager
• Assists in the admission process as requested
• Interprets and communicates Arkansas Hospice policies and procedures to nursing staff and others
• Coordinates and collaborates with interdisciplinary team members for patient care planning and management
• Updates the Plan of Care on an ongoing basis, ensuring accurate and timely documentation
• Facilitates effective communication with patients, families, and healthcare providers
• Monitors pain and symptom control to ensure optimal patient comfort
• Coordinates CNA scheduling needs with Patient Care Coordinator/Area Manager or designee
• Initiates referrals for hospice-related care and services
• Serves as a preceptor for nurses in orientation, students, and trainees
• Participates in interdisciplinary team meetings, case conferences, and other committee meetings as assigned
• Actively participates in Performance Improvement activities
• Participates in community outreach activities to educate the public about Arkansas Hospice services
• Coordinates and provides education on hospice and hospice services for patients, families, and the community
• Rotates on-call duty to provide 24-hour service to patients and families as assigned
• Maintains strict confidentiality at all times
• Utilizes and models excellent customer service skills at all times

Requirements

• Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred
• Minimum of one year nursing experience in a hospital, home health agency, hospice, or related program
• Licensed as a Registered Nurse (RN) in Arkansas
• Valid Arkansas driver's license, personal automobile, and automobile insurance required

• Knowledge of palliative care with emphasis on symptom control associated with terminal illness
• Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families
• Knowledge and acceptance of hospice philosophy and principles of care
• Skill in organizing and prioritizing workloads to meet deadlines
• Ability to recognize both positive and negative aspects of death and grief
• Ability to communicate effectively both orally and in writing
• Ability to communicate effectively with co-workers and other customers
• Ability to follow basic safety policies and procedures
• Ability to use good judgment and to maintain confidentiality of information
• Ability to work as a team player
• Ability to demonstrate tact, resourcefulness, patience, and dedication
• Ability to accept direction and adhere to policies and procedures
• Ability to work in a fast-paced environment

Working Conditions

This position is designated as a safety-sensitive position due to the regular handling of confidential and/or proprietary Arkansas Hospice and patient information, performing life-threatening procedures, monitoring and/or operating equipment used to perform medical procedures, and/or working with controlled substances and/or medicine. Any lapse of attention could have a significant impact on Arkansas Hospice and/or its patients, and could result in injury, illness, or death. For the safety and health of yourself and others, you must be able to work in a constant state of alertness and concentrate for long periods of time while performing life-threatening procedures, monitoring and/or operating equipment used to perform medical procedures, and/or working with controlled substances and/or medicine.
