The Northeastern Center seeks a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will work in partnership with child and family team members to develop a plan of care, oversee the implementation of the plan, and identify providers of services or family-based resources.

The ideal candidate will possess a bachelor's degree in psychology, social work, or a related field, and have a minimum of two years' experience in social work, therapy, or a related human service field serving Seriously Emotionally Disturbed youth ages 6-17.

Responsibilities will include facilitating county consortium meetings, monitoring services for participants, and ensuring care is delivered in a manner consistent with strength-based, family-centered, and culturally competent values.
