Hospice Care Coordinator

We are seeking a highly motivated and experienced Patient Care Manager to join our team at UChicago Medicine AdventHealth Hinsdale Home Health. As a key member of our interdisciplinary team, you will play a vital role in delivering exceptional patient care and support to individuals and families in need.
Responsibilities:
* Provide overall direction and coordination of multiple teams of clinical and support staff providing hospice and palliative care to assigned patients.
* Develop, implement, and evaluate interdisciplinary team goals and objectives.
* Maintain knowledge and communicate policies and procedures to staff regarding federal, state, and local guidelines as well as Medicare, Medicaid, and insurance reimbursement requirements.
Requirements:
* Minimum 5 years' experience as a Registered Nurse in hospice or palliative care setting, including three years supervisory experience.
* Active Illinois Registered Nurse license and AHA Basic Life Support Certification required.
* Strong leadership and communication skills, with the ability to motivate and guide team members to achieve exceptional patient outcomes.
