About the Role

We are seeking a skilled Patient Care Coordinator to join our team at CenterWell Home Health. As a Patient Care Coordinator, you will play a vital role in ensuring the seamless delivery of high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Develop and maintain relationships with key hospital, skilled nursing, and assisted living facility discharge planning services to provide ongoing updates on our services.
  • Conduct facility visits to assist our clinical team in determining patient eligibility and ensure effective communication with program staff and other field sales resources.
  • Actively participate in weekly program business development meetings, bringing relevant data, reports, and information regarding changes within accounts and referral sources.
  • Assist in the timely processing of physician orders as directed.
Requirements
  • RN/LPN/LVN license.
  • Strong knowledge of governmental regulations and Medicare eligibility requirements.
  • Comprehensive understanding of potential care plan needs for patients and coordination of necessary resources.
  • Excellent customer service, account development capabilities, organization, time management, problem-solving, communication, and selling skills.
Preferred Qualifications
  • Bachelor's of Science in Nursing preferred.
  • A minimum of three years clinical experience preferred.
