The Kodiak Community Health Center (KCHC) Clinical Nurse Specialist (CNS) plays a vital role in the collaborative process of delivering care and treatment to patients.

The CNS will evaluate, interpret, tailor, coordinate, and communicate the patient's care plan while working alongside healthcare providers and all members of the interdisciplinary health care team to manage and facilitate patient registries and care delivery appointments and services.

The CNS provides direct patient care while ensuring efficient patient flow within the clinic.

Key Responsibilities:

Functions as a member of the KCHC Healthcare Team in delivering health services to individuals, families, and communities.

Collaborates effectively with patients and their caregivers, respecting each patient's preferences, as well as with other members of the KCHC Healthcare Team to achieve shared objectives across various settings, ensuring coordinated, high-quality care.

Offers ongoing guidance, support, and education to other members of the KCHC Healthcare Team to ensure effective, quality primary care for KCHC patients and their families.


CARE COORDINATION:
Ensures continuity of patient care with patients and families following hospital admissions, discharges, and emergency department visits. Promotes clear communication among clinical provider staff. Assists patients in navigating the healthcare system by acting as a patient advocate. Facilitates patient education and access to healthcare and community resources.

Coordinates continuity of patient care with external healthcare organizations and facilities, including transitions of care such as:
hospital admissions, discharges, and referrals from primary care providers to specialists. Follows up with patients regarding lab and radiology results, and schedules follow-up appointments as necessary.

LABORATORY DUTIES:
Collects laboratory specimens, including phlebotomy, and performs CLIA waived laboratory tests.

CLINIC VISITS:

Conducts nursing visits that include, but are not limited to: urinary catheter insertions/changes, bladder scans, allergy injections, non-stress tests, administration of medications and vaccinations, IV insertions and administration of intravenous infusions, taking vital signs, performing EKGs, placement of heart holter monitors, wound care, glucose monitoring education, ear irrigation, and suture/staple removal.


TRIAGE FUNCTIONS:
Operates in the capacity of a triage nurse, providing in-person and telephone support to patients during office hours, which includes:


Assessment of symptoms for various medical issues with the ability to determine if the patient should follow up with a physician or seek urgent care.

Scheduling appointments based on telephone assessments.
Communicating patient information to the physician and following through with physician recommendations.

ASSESSMENT AND PLANNING:
Monitors patient conditions on a monthly basis or more frequently as indicated. Coordinates an evidence-based comprehensive action plan for all patients on the registries. Supports patient self-management. Manages patient registries to ensure compliance, timely lab and medication management, and patient appointment scheduling.

DATA COLLECTION AND REPORTING:
Responsible for the collection, reporting, and analysis of clinical data. Evaluates clinical care and resource utilization.

DOCUMENTATION:
Records patient care information and registry data in an accurate, concise, and timely manner.

PARTICIPATION:
Actively engages in staff meetings and other relevant meetings. Participates in professional, departmental, and unit-based education and competencies. Assists others in transitioning and applying knowledge to clinical and operational settings. Participates in team huddles. Promotes and models collaborative practice relationships with all healthcare professionals.

Recognizes the importance of cultural diversity among patients, families, employees, medical staff, volunteers, and community members in fostering productive and positive relationships.

Safeguards all forms (electronic, written, and oral) of confidential information related to patients, patient families, medical staff, and employees.

Complies with organizational policies regarding confidentiality, fraud and abuse, conflict of interest, and employee behavioral expectations.

Performs other related duties as assigned that support the overall mission and vision of the Kodiak Community Health Center.



Essential Job Qualifications:

Education:
Associate's degree in Nursing required. Bachelor's and/or Master's degree in Nursing strongly preferred. Current Alaska RN licensure required and must be maintained in an active status thereafter.

Experience:
A minimum of two (2) years of relevant clinical experience is required. Must have previous hands-on clinical experience and demonstrated clinical knowledge and competency. Must possess excellent computer and customer service skills with a compassionate approach to care.

Licensure/ Certification:
Must possess valid current licensure as a Registered Nurse in the State of Alaska, and current BLS certification.
