Palliative Care Specialist

About the Role:

We are seeking a highly skilled Palliative Care Specialist to join our interdisciplinary team at Mayo Clinic. As a Palliative Care Specialist, you will work collaboratively with patients, families, and healthcare teams to provide comprehensive care to individuals with advanced, life-threatening illnesses.

Responsibilities:

  • Provide acute hospital care to complex medical patients with a high symptom burden
  • Work collaboratively with the Palliative interdisciplinary team and consultant to deliver medical care to hospitalized patients
  • Coordinate discharge planning and facilitate the patient's transfer to the next phase of care
  • Consult and make referrals to other specialties and members of the healthcare team as appropriate

Qualifications:

  • Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program
  • Current Minnesota RN and APRN license and certified as an ANP, ACNP or FNP
  • Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice

Additional Qualifications:

  • Hospice and Palliative Medicine certification highly encouraged within 2 years of hire
  • Strong written and oral communication skills

Work Environment:

This position requires a rotation of days, nights, weekends, and holidays. Day shift length is 8 or 12 hours and night shift is 13 hours. This is a 24/7 service area. There will also be cross coverage responsibilities with the palliative consult service on occasion.
