Case Manager II, Acute Psychiatric

Sutter Health is seeking a skilled and compassionate Case Manager II, Acute Psychiatric to join our team. As a key member of our healthcare team, you will play a vital role in coordinating the care of patients with acute psychiatric needs. Your expertise and dedication will ensure that patients receive the highest quality care and support throughout their treatment journey.
Responsibilities- Coordinate patient care through clinical case management to ensure patients receive appropriate treatment and that admission, treatment, and discharge are well-coordinated.
- Collaborate with patients, families, physicians, treatment teams, external case managers, referring therapists, outpatient clinics, and payors to ensure seamless care coordination.
- Develop and implement comprehensive, cost-effective care coordination plans that meet the clinical needs of patients.
- Conduct initial and concurrent screenings to identify case management needs and develop transition plans for patients.
- Monitor and manage patient care to prevent unnecessary admissions or readmissions.
- Provide discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department.
- Actively participate in patient rounds and collaborate with interdisciplinary teams to ensure timely discharge.
- Maintain accurate and up-to-date records of patient care and progress.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Master's degree in Social Work or related field.
- 2 years of recent relevant experience in case management or a related field.
- Strong knowledge of healthcare delivery and case management within a managed care environment.
- Comprehensive knowledge of Utilization Review, levels of care, and observation status.
- Working knowledge of laws, regulations, and professional standards affecting case management practice in an integrated delivery system.
- Excellent communication, interpersonal, and negotiation skills.
- Ability to work independently and exercise sound judgment in interactions with physicians, payors, and patients and their families.
This is a per diem/casual position with variable shifts, including rotating weekends. The salary range for this role is $52.96 to $71.23 per hour.
