Case Manager II, Acute Psychiatric Specialist

We are seeking a highly skilled and experienced Case Manager II to join our team at Sutter Health. As a Case Manager II, you will play a critical role in coordinating the care of patients with acute psychiatric needs, ensuring that they receive the most appropriate level of care and support.
Key Responsibilities- Coordinate patient care through clinical case management, ensuring that patients receive the most appropriate level of care and support.
- Collaborate with patients, families, physicians, and other healthcare professionals to develop and implement comprehensive care plans.
- Conduct thorough assessments of patients' needs and develop individualized care plans to address their unique requirements.
- Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Identify and address potential barriers to care, such as social determinants of health, to ensure that patients receive the support they need.
- Develop and maintain relationships with community resources and providers to ensure that patients have access to the services they need.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Master's degree in Social Work or related field.
- 2 years of recent experience in case management or a related field.
- Strong knowledge of healthcare delivery systems, case management principles, and regulatory requirements.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
- Ability to maintain confidentiality and handle sensitive information.
