A rewarding opportunity awaits a dedicated and compassionate Physical Therapist Assistant to join our Visiting Nurse Association of Hanover & Spring Grove home care team. As a key member of our multidisciplinary team, you will play a vital role in helping patients regain their independence and improve their quality of life through personalized physical therapy programs.
About the Role:
The successful candidate will provide rehabilitative care for home health patients, assisting with ambulation, transferring, gait training, and home exercise programs under the supervision of a licensed Physical Therapist.
Key Responsibilities:
- Deliver Exceptional Care: Provide physical therapy services according to a personalized care plan, prioritizing patient comfort and safety.
- Evaluate and Support: Assess muscle strength, mobility, gait, and range of motion while assisting with equipment needs in the patient's home environment.
- Patient Education: Educate patients, families, and caregivers on therapeutic exercises and durable medical equipment usage to promote recovery and independence.
- Collaboration: Work closely with the Physical Therapist, attending physician, and the home care team to ensure comprehensive, coordinated care.
- Document Progress: Submit timely and accurate clinical notes in line with Medicare Home Health Guidelines and VNA policies.
Requirements:
- Graduate of a CAPTE-approved Physical Therapist Assistant program.
- Successful completion of the PTA certification exam.
- Active PTA certification in the Commonwealth of Pennsylvania.
- Minimum of 2,000 hours of experience providing patient-related services under the supervision of a licensed Physical Therapist (home health experience preferred).
- Current CPR certification.
