Job Description

As an LPN at BAYADA Home Health Care, you will be responsible for providing one-on-one care to our clients in their homes, utilizing evidence-based practice and a client-centered approach.

You will work closely with our multidisciplinary team to develop and implement individualized care plans that cater to each client's unique needs and goals.

Key Responsibilities:
  • Providing skilled nursing care to clients in their homes
  • Conducting thorough assessments and developing comprehensive care plans
  • Collaborating with other healthcare professionals to ensure seamless care transitions
  • Maintaining accurate records and documenting client progress
Requirements

To be successful as an LPN at BAYADA Home Health Care, you must possess a valid nursing license in the U.S. and have graduated from a qualified nursing program.

Additionally, you should have excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
