We are seeking a skilled and dedicated Physical Therapist to join our team in Morris County, New Jersey. As a member of our home health care team, you will play a vital role in helping patients recover from illness or injury in the comfort of their own homes.

About the Role

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work to develop a personalized treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate progress towards goals.

  • Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
  • Observe and report patient response to treatment and any changes in condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress towards goals.
Requirements and Qualifications

To be considered for this opportunity, candidates must possess current Physical Therapy licensure in the state of New Jersey and current CPR certification. A valid driver's license and vehicle insurance, as well as access to a dependable vehicle, are also required.
