Job Summary:

American Health Partners is seeking a dedicated and compassionate Licensed Practical Nurse to join our team as a Resident Care Coordinator. In this role, you will have the opportunity to make a meaningful difference in the lives of our residents while developing your leadership and clinical skills.

Responsibilities:

  • Coordinate resident care and develop individualized care plans
  • Supervise and train nursing assistants to ensure excellent care delivery
  • Monitor resident conditions and report any changes to physicians and supervisors
  • Provide direct nursing care to residents as needed
  • Collaborate with interdisciplinary teams to achieve optimal resident outcomes

Requirements:

To be considered for this position, you must possess a current State of Tennessee Licensed Practical Nurse license and a strong background in gerontological nursing. A minimum of 2 years of experience in long-term care or a related field is preferred. Excellent communication and interpersonal skills are essential, as well as the ability to work effectively in a fast-paced environment.
