About the Role

Tennessee Quality Care, a compassionate and patient-focused healthcare organization, is seeking a highly skilled Full-Time Physical Therapist or Physical Therapist Assistant to join our dynamic Home Health Division in Shelby County, TN. As a dedicated member of our team, you will play a vital role in delivering exceptional physical therapy services to patients, ensuring they receive personalized care that meets their unique needs.

Job Summary

We are looking for a motivated and experienced Physical Therapist or Physical Therapist Assistant to provide high-quality physical therapy services to patients in their homes. Your primary responsibilities will include:

  • Conducting thorough evaluations to assess patients' functional levels and develop individualized treatment plans
  • Providing evidence-based physical therapy interventions to promote optimal patient outcomes
  • Collaborating with interdisciplinary teams to ensure seamless care coordination
  • Maintaining accurate and comprehensive patient records
What You'll Do

As a Physical Therapist or Physical Therapist Assistant at Tennessee Quality Care, your day-to-day responsibilities may include:

  • Developing and implementing effective treatment plans to address patients' specific needs
  • Conducting regular progress assessments to adjust treatment plans as needed
  • Communicating with patients, families, and healthcare professionals to ensure informed decision-making
  • Participating in ongoing education and professional development to stay current on best practices
Requirements

To be successful in this role, you will need:

  • A graduate degree from an accredited physical therapy program (Physical Therapist) or a physical therapy assistant program (Physical Therapist Assistant)
  • Current licensure in the state of practice (Physical Therapist) or certification (Physical Therapist Assistant)
  • At least 2 years of experience in an acute hospital setting or equivalent (Physical Therapist) or 1 year of experience as a Physical Therapist Assistant (Physical Therapist Assistant)
  • CPR and First Aid certification
  • Reliable transportation and a valid driver's license
