- Ensure high adherence rates to our program guidelines.
- Provide education on diabetes management, exercise advice, and healthy eating habits to clients.
- Support clients in diabetes management and overall well-being.
- Maintain rapport and relationships with patients.
- Document client information.
- Bachelor's degree in nursing, health administration, or a related field.
- Active LPN license in NY or NJ (preferred).
- Three to five years of experience in nursing, health administration, or a related field.
- Experience working with diabetes patients.
- Certified Diabetes Educator (strongly preferred).
- Proficient in computers, Microsoft Office, and EMR systems.
- Excellent customer service skills and bedside manner.
- Strong communication skills (written and verbal).
- Ability to work independently with minimal supervision.
- Bilingual in Haitian Creole or Spanish (strongly preferred).
- Reliable transportation for commuting throughout NYC and/or NJ.
