We provide comprehensive services, treatment, and housing to individuals experiencing homelessness, substance use disorders, and serious mental illness.

We are seeking a Crisis Intervention Professional to join our Mobile Response Team as a Case Manager. In this role, you will work closely with an interdisciplinary team to provide crisis intervention services, complete risk assessments, develop safety plans, and coordinate care with clients and housing staff.

As a member of our team, you will have the opportunity to work with a dynamic and compassionate group of professionals who share your passion for delivering high-quality services to vulnerable populations. You will also have access to ongoing training and professional development opportunities to enhance your skills and knowledge.

Responsibilities:

  • Provide crisis intervention services to individuals experiencing mental health and/or substance use crises
  • Complete risk assessments and develop safety plans with clients and housing staff
  • Coordinate care with clients and housing staff to ensure continuity of services
  • Work collaboratively with an interdisciplinary team to deliver comprehensive services to clients
  • Document client interactions and services provided in accordance with agency policies and procedures
