Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the homes of our patients, identifying potential health risks and developing personalized care plans to improve their overall well-being.

Key Responsibilities
  • Conduct thorough health assessments in patients' homes, including medical history, physical examination, and review of medications
  • Identify potential health risks and develop personalized care plans to address these risks
  • Collaborate with patients, family members, and healthcare providers to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's policies and procedures
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills

Travel Requirements

This role requires frequent travel to patients' homes, as well as occasional travel to Matrix Medical Network's offices. A valid state driver's license and adequate automobile insurance are required.
