Job Overview

We are seeking a skilled Medical Care Specialist to join our team at Abode Care Partners. As a primary home health services provider, you will deliver personalized medical care to seniors and patients with complex needs in their home or assisted living facilities.

About the Role
  • Plan and evaluate patient care to ensure accurate diagnosis and effective treatment of chronic illnesses.
  • Treat acute and episodic conditions, including minor injuries and accidents.
  • Collaborate with healthcare professionals to maintain a comprehensive plan of care and improve patient outcomes.
Key Responsibilities
  1. Evaluate patient medical needs and develop individualized care plans.
  2. Conduct thorough physical examinations and gather medical histories.
  3. Prescribe medications and treatments as needed, while ensuring compliance with state regulations and HIPAA guidelines.
  4. Document patient interactions, diagnoses, and treatment plans accurately and efficiently.
Requirements
  • A Master's degree in Nursing (MSN) from an accredited institution.
  • An active state Nurse Practitioner license, current DEA licensure, and National Board Certification.
  • At least 5 years of nursing experience, with a minimum of 2 years as a nurse practitioner.
  • Excellent interpersonal communication skills, both verbal and written.
  • Strong knowledge of HIPAA and OSHA regulations, as well as proficiency in electronic medical records systems.
Compensation and Benefits

Abode Care Partners offers a competitive salary range of $100,000.00 - $120,000.00 per year, along with a comprehensive benefits package that includes:
