- Conduct thorough assessments, identify patient needs, and prioritize interventions to achieve optimal outcomes.
- Administer medications, perform wound care, and provide education on disease management and self-care techniques.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and coordination.
- 1-2 years of RN acute care experience required.
- Valid NYS RN Licensure.
- A comprehensive orientation to home health care, ensuring a smooth transition into your new role.
- A supportive work environment with access to clinical nurse specialists, in-depth computer training, and assistance with insurance authorizations.
- A competitive pay range of $36.50 - $49.95, based on experience, skills, and education.
