Case Manager Job Description

At Hamilton Families, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in providing personalized support to families in our shelter program.

Key Responsibilities:

  • Offer case management services to families, including intake, assessments, and tailored service plans.
  • Connect families with vital community resources, advocating for housing, employment, and support in areas like mental and physical health, domestic violence recovery, and family reunification.
  • Design and facilitate dynamic workshops that empower families with essential life skills and strategies for crisis intervention.
  • Build strong networks with community services to ensure families receive the support they deserve.
  • Work closely with case managers, supervisors, and community partners to provide seamless and effective support for families.

Requirements:

  • 1+ years of experience in social work, psychology, human services setting or related field.
  • Familiarity with Bay Area resources for low-income families.
  • Bilingual abilities, especially in Spanish.
  • Highly proficient in Microsoft Office Suite, Salesforce, and other software.
