Job SummaryWe are seeking an experienced Hospice Nursing Assistant to join our team. As a key member of our interdisciplinary team, you will provide high-quality patient care, focusing on comfort, dignity, and quality of life for our patients and their families.
Responsibilities- Deliver patient care, including personal care, nutrition, elimination, safety, and special procedures.
- Participate in interdisciplinary team conferences and special case conferences.
- Document patient information accurately and timely, adhering to HPCCR guidelines and licensure standards.
- Assume responsibility for maintaining a clean and safe environment, adhering to facility cleaning procedures.
- High School diploma or equivalent, plus training/certification in a related field.
- Certified Nursing Assistant (CNA) certification, with current listing status.
- Minimum 1 year experience in home care, working with terminally ill and/or geriatric patients.
