SSM Health is seeking a compassionate and detail-oriented Hospice Care Assistant to join our team at our Hospice Home Monroe location.

As a Hospice Care Assistant, you will play a vital role in providing high-quality care to our patients and their families. Your primary responsibility will be to assist with medication administration, ensuring accuracy and compliance with state and federal regulations.

Under the direction of a licensed nurse, you will work closely with our interdisciplinary team to provide whole-person care, addressing the physical, emotional, and spiritual needs of our patients.

Key responsibilities include:

  • Maintaining accurate medication inventory and ensuring proper distribution to residents
  • Providing quality care to subacute and chronically ill residents, assisting with acute care as needed
  • Administering medications according to standards of care and maintaining competency in required skills
  • Assisting with transfers, ambulation, and meal service as needed
  • Participating in report and communicating with the team nurse and other care providers

Required qualifications include a high school diploma or equivalent, one year of geriatric experience, and current certification as a Basic Life Support Healthcare Provider (BLS HCP) and Medication Aide Training from TMU Wisconsin. A Certified Nursing Assistant (CNA) certification from the Wisconsin Department of Health Services or the Wisconsin Nurse Aide Registry is also required.

This is a night shift position with a scheduled weekly hours of 24. We look forward to welcoming you to our team
