Job Summary:

As a Hospice Care Assistant with SSM Health, you will play a vital role in providing compassionate care to patients in a home health and hospice setting. Your primary responsibilities will include assisting with daily living activities, such as bathing, ambulating, and feeding, as well as monitoring patients' behavior and physical condition. You will also be responsible for collecting and documenting data for patient assessments and providing information to licensed nurses as needed.

Responsibilities:

Assist patients with daily living activities, such as bathing, ambulating, and feeding
Monitor patients' behavior and physical condition, reporting any changes to licensed nurses
Collect and document data for patient assessments
Provide information to licensed nurses as needed

Requirements:

Completion of an approved nurse aide training program and passing grade on state of work location required exam
No experience required

Work Environment:

Home health and hospice setting
