**Overview of the Role**

D-H Lebanon-MHMH's Department of Psychiatry seeks a full-time licensed and Master's prepared behavioral healthcare professional to join our expanding pediatric collaborative care team.

This position will be based in the general ambulatory pediatrics clinic at the main hospital as well as the primary care clinic at Heater Road in Lebanon, NH.

As a member of the Collaborative Care team, you would have the opportunity to work closely with pediatric primary care providers, child psychiatrists, and other clinical staff to care for children and adolescents with behavioral health concerns directly in the primary care setting.

**Key Responsibilities:**

  • Triage incoming referrals and conduct initial evaluations to clarify behavioral health concerns.
  • Develop individualized care plans and utilize evidence-based treatment protocols to deliver care.
  • Provide brief, evidence-based behavioral interventions such as behavioral activation, motivational interviewing, and parent skills training.
  • Support psychotropic medication management as prescribed by medical providers.
  • Communicate effectively with patients, families, primary care providers, and the consulting child psychiatrist.

Required Qualifications:

  • Master's degree in Social Work, Mental Health Counseling from an accredited institution of higher education.
  • Licensed or license-eligible as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
